Simple Theme v 3.1.1.0

When adding text using the "Insert/edit code sample" WYSIWYG button, it looks great in the editor but fails to use the correct css when published. In the example below, I'm using language-bash.

Steps to reproduce

Use the Insert/edit code sample button to add a code sample, selecting a language for syntax highlighting. Click Preview to see it non-syntax-highlighted.
