You can already open in new tab by middle-clicking or Ctrl+clicking the link.

It can be an option. The app will check the existing note tab. If the note is already open, it will jump to there. If not, it will open as a new tab to prevent replacing the current note.

I would avoid creating a new navigation paradigm unless it's common practice.

No, it is an Obsidian plugin. Here is the link: https://github.com/Raphlette/obsidian-new-tab-plus. Maybe use pin note to lock the note to prevent replacing it?
