Add the ability to configure which prefetch data will be honored

Since many client implementations intentionally omit certain prefetch information and instead require that services request additional FHIR data to flesh out card contents, it would be great to simulate this scenario in the sandbox. I'm imagining something configurable in the sandbox so a service could quickly test both prefetch data being honored and omitted without having to redeploy their discovery endpoint.
