Signature Checker: Review `ElementUtils.getQualifiedName` uses

The declaration and many uses suppress Signature Checker warnings.
Review what the possible strings are and either refine the annotations or split the API to have cleaner usages.

TODO: should it be @CanonicalNameOrEmpty because of the default package and anonymous classes?
