dl_allvers: handle questionnaires with different names but same questionnaire variable

dl_allvers function currently matches the questionnaire Id to find all the versions of a questionnaire. However, questionnaires with different names might have the same questionnaire Id and variable but the user might prefer to match by questionnaire name instead of Id for their purposes
