Add code coverage instrumentation for the test suites

There is currently no coverage tooling configured (no nyc/c8/istanbul in package.json), so we cannot quantify how much of the codebase the unit/integration/e2e/smoke suites actually exercise. This makes it impossible to track coverage of the core API pillars
