add h5 load and save to io

turns out slicer now requires deformation and vector files in h5 format for their transform module. ants outputs is nifti. any chance nibabel will become an elegant bridge solutionl?

hi matthew, andras was the slicer support responder for this issue (copied here). hopefully he can flesh out details. andras, i inquired about adding .h5 support to nibabel to extend support for Slicer and to leverage .h5 support for massive datasets in nibabel.
