Use “SNOMED CT” instead of “SNOMED” in global standards list #135

Description

@vivek-jais

Hi,

While reviewing the website, I noticed a small terminology detail on the Core section.

Current text:
"Built on global standards: FHIR R5, SNOMED, LOINC, UCUM, ICD-10; terminology server; BYOT."

Suggestion:
Since the official name of the standard is SNOMED CT, it may be more accurate to write:

"Built on global standards: FHIR R5, SNOMED CT, LOINC, UCUM, ICD-10; terminology server; BYOT."

This is just a minor documentation/terminology improvement to reflect the official naming of the standard.

Thanks for the great work on the project!
