Make an integration manager

I thought I'd filed an issue about this, maybe I have but I can't find it.

Integrations aren't in the spec but since Element supports them, MSC1956 has some description that we can base it on

This should meet our use-case/needs while being an open source integration manager to succeed the now archived Dimension
