Not all listed schemes work (eg. phd)

Hi,

I've noticed some of the schemes listed at https://github.com/chriskempson/base16-builder/tree/master/schemes don't seem to work.

One example is the "phd" scheme, but there are others too.

hypersixteen depends on the base16 npm package, so it naturally only supports the schemes that are available in that package. I'll look into adding support for schemes not originally supported.
