Use coder ssh in place of coder vscodessh #420

Build on top of recent coder PRs to make coder ssh provide equivalent functionality to vscodessh, avoiding the need for a VSCode-specific ssh subcommand.

This aligns the VS Code extension with the proposed change to the JetBrains extension in coder/jetbrains-coder#521.

Depends on coder/coder#16078, coder/coder#16080, coder/coder#16088

@bcpeinhardt: No rush on this since it's still waiting for the CLI release, but I'm happy to address any review feedback you have and get ahead of that...
