Calculated Access measures, updated metadata and refined scripts #298 and #297

Updated the last modified date and added detailed descriptions for data sources, processing methods, and limitations. Included new sections on travel time metrics and geographic boundaries.

@ywkim312 , once Adrita updates the registry you'll be ready to merge the new HIV and HCV files here -- they are in a folder called "data to merge" in this PR. Okay to drop them once you've merged.

Ah, actually -- @adrita13 you'll need to add the new time periods and source information to the CMS registry. Once that's done, the PR will reflect the changes on the backend, and then ready for Yong Wook.

@adrita13 I removed 2018 and 2019 metadata notes as these are all new.

Question: should these be census files from 2020, not 2018? Please check the metadata and update. If you used 2025 resources with the older census files, may need to update..

Currently, I don't see table sources linked for the below measures, so they are all new variables from my understanding.
