The logic branch where this array was left uninizialized was when writing individual Image objects with FITS compression, not MaskedImages, so it won't have affected any production use cases.
The "U" is for "universal". We can't call it ZSCALE because astropy strips that from headers before we'd have a chance to interpret it, assuming it's a mistake (ZSCALE is only in the standard as a column name, not a header card).
