docs: make skill docs self-contained - #12

This repository is public. These pages carried inline pointers to design documents that live in a private repository — identifiers a reader here cannot resolve, and which describe internals rather than behaviour. Removed the pointers. Nothing else changes: each sentence already stated the behaviour on its own terms, so the surrounding text stands unedited and no guidance is lost. Follow-up to the same cleanup applied to xgrep-rule-creator in #11; this covers the remaining pages. Co-Authored-By: Claude Opus 5 (1M context) <noreply@anthropic.com>

documents that live in a private repository — identifiers a reader here cannot
resolve, and which describe internals rather than behaviour.
