This PR adds a script to examine primary care encounters over time by migration status where the denominators are all individuals who have each respective migration status (migrant, non-migrant, unknown) and meet the general inclusion criteria. The migration status variable is created using the migration_status_variable.py script.
Because I want to capture any planned encounters, regardless of whether they actually occurred or not, I am including appointments with a seen_date or start_date within the given interval.
