claude: add fork PR support to code review workflow#4099

Add pull_request_target trigger with dual-trigger architecture so fork PRs can access secrets for Claude review. Internal PRs continue using pull_request; fork PRs use pull_request_target. Extend file modification guard to also block .github/workflows/ changes.
