📋 Description

Add breaking changes and version specific upgrade notes for Umbraco 18 (the main updates being in 18/umbraco-cms/fundamentals/setup/upgrading/version-specific/README.md).

Also updates various existing pages to reflect these updates for 18, and links to the 18 version of the API documentation.
