chore: improve review skill

Changes

This PR improves the code review skill by steering the agent more towards the gathering of business requirements, and assessing them. It allows the agent to navigate PR, comments and issues so it can collect and build business requirements and context.

It also rewords the skill so that it's completely fine not finding anything, and changes the threshold of the PR. The threshold is more focused towards the business requirements.
