New schema

Update the schema to match the 'Location of death' updates that are behind the `burial_location_of_death_updates' feature toggle.

To Do

  • Update vets-api so that forms can be submitted with new schema
  • Update vets-json-schema package version when PR is ready to be merged and feature can be enabled in production
